Imaging: The only way to know if you have a thymoma is to get tissue. There are often detected on imaging like a ct scan. When found, they are often resected either through a sternotomy or minimally invasively by thoracoscopy or robot. Sometime you can biopsy these, but their location often make them not accessible to a needle biopsy.

CT biopsy: Or direct to surgery if suspicion is high enough.

Very unlikely: Thymomas rarely spread via lymph nodes. Most often they spread by direct extention into other organs or by "drop" metastasis into the pleural or pericardial cavities.

Depends clinical set: This depends on the clinical setting. Sometimes you may need surgical excision, especially if concerned about malignancy. Biopsies can be done. But surgery is the usual approach. Depending on the spread even chemotherapy is indicated. It is associated with myasthenia gravis at times. At times removal can cure the myasthenia gravis.
