History & Sodium: If clinical history suggests water intoxication, serum levels of electrolytes, especially sodium are tested to check of dilution of blood. In addition serum osmolality may also be tested. Since water intoxication may produce altered mental status other tests, e.g., alcohol, substance abuse, diabetes etc may also be done.

History, phys, labs: History of excess "free water" intake; physical may have altered vital signs, mental status, focal neurological abnormalities. Labs include serum and urine electrolyte (sodium, potassium) levels, and concentration (osmolality).
