Very hard to do: The bowel is somewhat elastic, which is what allows it to expand to accomodate food, stool and gas. The surface is also very slippery. The end of thermometer is blunt and smooth. It would be very hard to perforate the rectum with a thermometer unless a person was using deliberate force. If so much force were used that it actually perforated the rectum, the infant would be extremely distressed.
