You can wait: Grade 3 AC separation is always the tricky one. Recently treated one conservatively and the patient had full motion without pain and good strength. His complaint was that sometimes his shoulder clicked. Many treated surgically can have other problems. I'd try to treat it conservatively and if you still have problems affecting function, you can reconstruct it.
