Chronic fissure: Initial chronic fissure treatment includes correcting any constipation or diarrhea along with medicated creams (nitroglycerin ointment and/or diltiazem). The meds are directed at physiologic relaxation of the internal sphincter. Should the fissure does not heal. despite these measures, then sphincterotomy is the next step and should allow healinge. Would recommend seeing a colon & rectal surgeon.
