Could be many things: As a cashier, you are probably firmly pressing down on your index finger often. Over time, this can lead to tendonopathy such as trigger finger or arthritis. Consider getting evaluated by your doctor or a hand surgeon.

Great Q 4 OrthoHand: I assume that someone with intense or recurrent pain would have notified a local doctor. This sounds like a condition associated with repetitive motion or overuse and is good for an orthopedic hand surgeon to address also.
