Surgery?: This question has to be answered in the context of individual high risk patients. In general there has to be a significant benefit to be expected from risking surgery on an end stage anything patient. If perhaps repeated CNS episodes attributable to the carotid are happening and antiplatelet therapy isn't helping, that might be a reason to risk surgery. Considerations need to be individualized
