How accurate are blood tests for appendicitis?

Not diagnositic: Blood test do not diagnose appendicitis, elevated blood cell counts in a person with right lower quadrant pain support the diagnosis. Imaging studies are more reliable than blood tests.

ReliableMostOfTime: While most cases there is elevation of white blood cells&high neutrophil count&that along with a typical history&exam one can pretty well clinically diagnose Acute Appendicitis&that's what we did in past before sophisticated imaging was available&I practiced in that period in sixties&seventies. But now we like to confirm the diagnosis by imaging.Cat Scan is more reliable but often we do ultrasound
