CDC's ADHD page: http://www.cdc.gov/ncbddd/adhd/diagnosis.html it gives a list of the diagnostic criteria for adhd. Most of the paper tests that we use put numbers to each of those criteria in order to determine severity. This is a decent online test, but it isn't complete:. http://pediatrics.about.com/cs/adhd/l/bl_adhd_quiz.htm if you combine it with the cdc page it would be fairly accurate.

CHADD : The online "tests" for add are really questionnaires that screen for it. The only sure way to be tested for adhd is to undergo a neurodevelomental evaluation by a well trained doctor. If you fill out an online add test that indicates you may have it, consult your doctor for confirmation and referral to a specialist who deals with this every day.
