Interesting question: In medicine i think of a "sign" as an objective observation by the examiner, vs "symptom" as a subjective self observation by the patient. There fore the sign of exercise intolerance would be the patient "failing" an exercise stress test with a critical result.

Think along the: lines of more shortness of breath and/or fatigue than you would otherwise expect at any particular level of exercise(exertion).
