Mouth symptoms: The most common symptoms of apple allergy or oral allergy syndrome with apple, are itching, burning or stinging of the mouth, tongue, lips or throat. Sometimes swelling may occur. In more severe cases there may also be nausea, vomiting, diarrhea, abdominal cramping, and rash. Rarely anaphylaxis may occur.

Mouth itching: Most apple allergies occur in patients with pollen allergies and only cause mouth or throat itching when eaten raw, but not when cooked. Less commonly, patients could be allergic to both raw and cooked apples, and present with more serious symptoms, such as throat swelling or trouble breathing, or hives.
