Ear exam: Hearing loss could be subtle and the best way to determine is through audiometer testing. Have your GP refer you to a ENT specialist who can help assess you further. You ear wax could also be cleaned at the same time.

Earwax: I think its due to the ear wax - attenuating the sounds. I would avoid using Q tips to clean your ears. Your family doctor should be able to flush your ears out if needed and that should take care of the problem. You can try warm water and hydrogen peroxide mixed 50/50 to help loosen the wax.
