Clear fluid at first: The fluid in a cold sore initially is more watery, like a small water blister, usually a cluster of small bubbles, and then this can become more milky and then it usually dries and crusts over. A pimple usually starts as a red bump or papule that then forms the classic pimple with the yellow, thick fluid.
