Stress tests do not necessarily pick up occult or "sublicinical" atherosclerosis. That you use tobacco is a a huge risk factor for cardiovascular disease. If you could quit, you would do yourself a huge favor from a CV point of view, and your lung function too would normalise after 1 year to that of a non-smoker.
