Many things: but the most concerning is heart related. If you have heart disease, diabetes, high cholesterol, smoke, and family history of heart disease, I would seek medical attention ASAP. If 46 years old, I suggest a check up as well as blood tests/EKG (especially if you are having pain). Other potential causes include muscular pains, spleen issues, nerve impingement to name a few.
