They act on receptor: We have our own built-in opioids (endorphins and enkephalins, and others) for which there are a bunch of receptors - mu, kappa, delta, sigma and epsilon. Opioids may have preferentially at one or more of these, and they will cause dysregulation of those receptors, to where the person requires more and more of the opioid to obtain the same effect, as well as withdrawal symptoms when they run out.
