See doc: Strep infections can present without a sore throat or runny nose. There are other causes. Fever for 4 days needs to be checked, but do make sure you get a throat culture. Less than half of the cases of strep infections in adults present classically with sore throat. Lots of docs are rigid about complying with guidelines and won't culture without it and miss strep in both kids and adults often.
