4 months or 4 years: old? The posterior fontanelle or " soft spot" usually closes by 1-3 months. The anterior fontalle closes between 9-19 months. Delayed closure of a fontanelle may have a medical cause. A sunken fontanelle is usually from dehydration, rarely from malnutrition in the US. His pediatrician can review his medical & developmental history & do physical & neurological exams to determine significance.
