Yes: Often patients can get relief with Neurontin or Lyrica (pregabalin). These help with neuropathic pain.

Not sure...: Without looking at the skin it's not easy to say with certainly what will help. If there is infection that has developed in the area where the shingles outbreak occurred antibiotics might be needed. Usually we treat pain immediately to prevent post-shingles nerve pain, but four weeks is too late. Gabapentin and ultram may help. But he will need a doctor to look at him and figure out what he needs.

Shingles pain: Providing that no other significant health issues are present (such as liver or kidney disease), Tylenol (acetaminophen) or Motrin can be used for pain. If these over the counter pain medications are not sufficient, prescription pain medications are available through a primary care provider.
