Possibly eustacean: tube dysfunction. Many individuals get a little flushed and congested in the sinuses when exercising, and this could cause partial blockage of the Eustacean tube, just like ears popping on airplane or with a bad cold. Also, if you clench your teeth with exercising, this can cause some TMJ discomfort, and that joint is near the ear.

Depends: Are you using earbuds or earphones when you work out? This is the most common reason; however, it may also be due to an ear canal irritation or infection. I suggest a checkup to be sure.
