Good luck: Without a name of clinic and the name other doctor and the name of the state you will need an investigator rather than a Doctor to find him

Google . . .: Time to play detective (and hope s/he hasn't moved - if they have, you're outta luck). I'd just search for urologist in whatever town you were treated. Assuming you didn't live in one of the larger metropolitan statistical areas, there can't be that many urologists in that town. Even so, it's a finite number. If you don't recognize doctor's/clinic's name, check out listed locations. Good luck!
