Google . . .: Time to play detective (and hope s/he hasn't moved - if they have, you're outta luck). I'd just search for urologist in whatever town you were treated. Assuming you didn't live in one of the larger metropolitan statistical areas, there can't be that many urologists in that town. Even so, it's a finite number. If you don't recognize doctor's/clinic's name, check out listed locations. Good luck!
