OTC: Those are the instructions on the OTC meds so that people don't get on the meds for long term without seeing their doctor. That could mask a more serious problem and delay getting help until the issue is much more serious. If you need heartburn treatment more than 14 days it is a good idea to consult with a GI specialist.
