Taking bp in both arms, sometimes it's extremely close & others it's a difference of 10-18 points (and even varies which arm is the high one). worry?

Variance: The pressure should be similar in both arms but since the difference isn't always present,it doesn't appear to be a fixed blockage in an artery of the arm with the lower reading. Perhaps the varience is due to slightly different techniques in your BP measuring. The higher reading is the one we go on,however.
