Many possibilities: Dermoids are frequently bilateral so that would be the first possibility. It could also be a hemorrhagic corpus luteum or an endometrioma. The ultrasound characteristics can help with this differential as can Doppler flow studies. Your doctor should be able to tell you more what the probabilities are. Good luck.
