Perineal lump: I am not clear where the lump is - but I am suspicious of prostate area and prostatitis, given symptoms. This is highly possible; also diabetic men can become infected in skin areas of scrotum and beneath because of plugged hair follicles; this could be present; he needs an exam asap and likely needs antibiotic treatment; he can bathe for comfort, too
