IBS: Many people with IBS have pain with defecation, which can be due to irritation of the rectum from liquid stool, or from swelling from too hard stool. With liquid stool, the tissue is exposed to potassium in the stool, with hard stool the veins in the rectum can swell causing hemorrhoids. Normal stool does not cause irritation to the rectum.
