Exam: You will need a comprehensive physical exam and metabolic workup. I would suggest consulting a psychotherapist for cognitive behavioral therapy and you physican for adhustment of you antidepressant medication.

Psychiatry: I would start with psychiatry to deterimine if your insomnia is related to depression / anxiety or if there is another diagnosis. The psychiatrist would also be able to adjust and change your medications to treat the mood disorder and facilitate sleep. Poor sleep will always make mood problems worse. I would also try taking melatonin 5 mg a night for 2 weeks to see if that helps.
