Probably fine: One possibility is that if a light source is on one side of the room, one pupil will have the appearence of being larger than the other due to the variation in light. Then with the light source, they are symmetrical. We are generally looking for the " red reflex" in babies for the most part on exam to rule out retinoblastoma, in which there is no " red reflex" and the retina will appear white.

Yes: The important part of a pupil exam is that both pupils react to light and communicate that with each other and that there is a good red reflex bilaterally. Often the 2 pupils are not quite equal in size.
