CHF & Spinal Issue: My advice will be you can work or do activity with some restriction. Dicuss with your orthopaedics or spine surgeon who has done surgery for more detail. Thanks.

Work capacity: It is not the matter of whether you should be working or not. It is based on how severe of a problems you have and how you tolerate the work and the consequences of type of work you do..

Work: How symptomatic are you? I have known a paramedic who continued to function with congestive heart failure and depending on your job many people get a spinal fusion to function. If you are having specific problems with your occupation you need to talk to your doctor or see a physician who does occupational medicine.
