Depends : First off, your doctor will have to talk to and examine you to determine if one is medically necessary. If you have insurance, the insurance companies are pretty adamant about needing to have a legitimate reason to have one done. Depending on the kind of insurance you have, you may not have to pay extra for it at all. You can call your ins company and ask them if there is a co-pay. If no ins, $$$
