Should be safe: The electrical stimulation from a TENS unit should not affect the heart rhythm, if TENS unit is used on the lower back, or further from the heart.

Probably: The current in the TENS unit is very minimal and unlikely to cause an arrhythmia , HOWEVER I would discuss it with your cardiologist before using near your heart.
