See a urologist: Testicular torsion usually is so painful that it is unlikely you could sit long enough to write a Health Tap question. The reflex you mention is variable and is not an indication by itself of testicular pathology. Try some hot baths. If this is not effective in relieving pain and/or pain persists ask your PCP to refer you to a urologist.

Seek care now...: You need to be examined to determine the cause of the testicular pain. Lack of cremasteric reflex is concerning. Please get evaluated at the ER now. Delay is treatment for some causes of testicular pain could result in the loss of a testicle.

Testicular Torsion: Although testicular torsion is typically associated with more intense pain, all medical problems have variations in presentation and differnent diseases can at times have common presentations. It is definitely not common for the pain to be a dull discomfort but given the risk of the disease in question it is best to have an ultrasound to know for sure.
