Muscle spasm: This is usually from a muscle spasm. Try stretching and/or icing the area. However, 4 years of pain in the same spot is unusual. Consider seeing a chiropractor or physical therapist

Chronic tendinitis: 4 years is unusual for tendinitis but not impossible. tendinitis/myositis is tender to the touch. massage is best with the addition of anti inflammatory agents. Winter green (used in BenGay) is good to use when you do the massage.
