Amikacin dose: Unfortunately there is more that I don't know about this patient than I do know. Amikacin is dosed based on a patient's kidney function and that is not provided. Assuming labs are normal, dosing would be 7.5mg/kg every 12 hours. I am questioning if the weight is correct. A 19 y/o female weighing only 41 lbs? If true this patient would receive 150 mg every 12 hours.

Depends on reason: Pediatric dosing is usually 15-30 mg/kg/day, divided in 1-3 doses a day, so total daily dose 300-600mg for someone weighing 44 lbs. Low or high dose depends on the severity of infection. Adult dose for uncomplicated UTI is only 250 mg twice a day, and max adult daily dose is 1.5 grams a day.
