Urgent care: An urgent care would be fine if they have X-ray. If not, then ER.

YES: YOu want to make sure there no Fracture. Usually with sprain swelling remains the same after 24 hours, unless you walk on it. If pain is very much, you are not weight bearing on this foot and it is doubly swollen you should get Xray of the Ankle. And if it turns out to be Sprain, you will get special shoes to support your ankle and healing will be faster as Ankle needs to be immobilized

Sprain vs. fracture: If it's that swollen, it probably will have to be x-rayed. If it's fractured, the treatment will be different in most cases then if you just have a sprain. If you can't get in to see an orthopedic surgeon within a day or two then the emergency room is probably your best bet. Ice and elevation and nonweightbearing on crutches is recommended in the interim.

Perhaps: in the ER we use a criteria to determine if ankle trauma requires xray or just splinting. At the end of the day, we end up doing xrays on most ankle pain regardless. Generally, if you cannot walk on it with all your weight, we will xray it. If the xray is negative, RICE protocol (Rest, Ice, Compress and Elevate).
