Not in everybody: but yes, after repeated contact with irritating substances, whether tobacco smoke or air pollution or others, the normal cells start to experience abnormal growth, called metaplasia, which may turn into malignant growth, or neoplasia. This does not happen in all smokers, and the metaplasia does not become malignant in all smokers, but certainly does in many.
