Many things: Did this happen slowly or rapidly? Is the lid red and painful? Is the lid itchy? Is the eye underneath red and is the vision altered? These are the sort of questions your ophthalmologist will ask to determine the cause of your puffy lid. If the answer to all of these is negative and the onset is rapid within one day or less, it is likely an allergy or insect bite.
