Rarely: Most time the problem is poor hygiene. Not brushing and flossing and seeing your dentist regularly. Amazingly how people don't have cavities till they see their dentist. Truth is, they don't know they have them till an expert can diagnose them. Weak teeth is usually an excuse for not taking proper care.
