Perhaps: you are describing cervical disc disease with radiculopathy. In my practice I have seen very high uncontrolled blood pressures that was secondary to cervical disc herniation. A good history and EMG- Nerve conduction test with an MRI cervical spine will give clinch the diagnosis - this easy. What to do about it is often harder. Consult your PCP to see if you need to see Neuro/sx specialist.

Possible: It the arthritis and inflammation or vertebrae condition is enough to be causing issues with the posterior circulation to the brain via the Vertebral Artery(ies),... when you extend the neck, the circulation can be compromised leading to dizziness, and with forward flexion the pressure would be relieved.
