Vaginismus: Involutary vaginal constriction: many causes, stress, anxiety, fear, pain etc. I would advise spending at least 20-30 minutes of gentle foreplay until both you and she are dripping wet. Verbal communication is quite importantant: ask what feels good or pleasant- penetration should wait for at least a month. Slow, careful, thoughtful, stimulating foreplay...and comminication should help.

Needs exam: I highly suggest you/she see your primary care doctor or ob/gyn and have a vaginal exam to check for any abnormalities that could be causing such pain, wounds, and bleeding. That is not normal.
