Swollen testicles: Swollen scrotum could be from hernia, hydrocele, tumor, infection, lymphatic blockage, torsion (twisting of testes blood supply) etc. Some of these causes are serious or emergencies. I would encourage him to get care real soon, at least at an urgent care center.
