I would pierce it: Ith a sterile instrument to allow the fluid to expel, but never deroof it. If the patient was diabetic or immunocompromised I'd recommend they see a foot doctor or dermatologist.

Protect it: If blister is still full you need to apply antibiotic ointment and protect it with a bandaid and padding. Once pressure is reduced the blister will reduce and dry out and the new skin can form over the raw skin.
