What does 2 out of 4 mean as far as reflexes are concerned as another set of reflexes i had 5 out of 5 as i was being tested for back pain and weaknes?

Reflexes : In doctor's notes, 2/4 reflexes reflect a normal reflex at the location tested. Higher numbers indicate brisker than normal, and lower numbers indicate less.
