Inflammatory vs. not: Can produce similar symptoms, but PMR is an inflammatory condition for which there is a single and effective treatment. FM is a condition with less defined symptoms which variably responds to a variety of interventions. Because they are common sometimes people will have both conditions.

Many differences: Polymyalgia rheumatica is a chronic inflammatory disease that is marked by muscle pain around the shoulders and hips. It can be associated with inflammatory changes of the blood vessels, which can place a person at risk for blindness. It tends to affect elderly persons. Fibromyalgia is also a chronic pain syndrome, not frankly inflammatory, tends to be more widespread and affect younger persons
