Involuntary?: If urine is being released involuntarily then different causes such as urinary tract infection, or medical conditions such as stress incontinence could be causes that could be adequately diagnosed and treated by a doctor. If she is doing it voluntarily, it's because she wants to.

Not abnormal: A spasm is occurring during orgasm, this increased pressure causes a small amount of urine to "squeeze out". Not terribly abnormal. My guess is that she has had a child or two.
