Usually not: Hi. With major trauma, hopefully the person is in a trauma center with good surgeons and bovies! We generally do not like epi in exrtremities because it's a vasoconstrictor and causes risk of limb ischemia. An early lesson in med school: don't use epi in penis, nose, fingers, toes. Higher up on extremities is safer, but with good trauma surgeons, who needs it?! And who wants to lose his, uh, NOSE?
