Could be either: Kidney stones are often extremely painful, with the pain coming in waves and do tend to be felt in the flank and back. A kidney infection would likely be causing fever and/or chills with burning of urination,although this can occur with stones too. Both can have blood in the urine, nausea, and vomiting. Best to see your doc and run some tests to rule these out soon.

Back/flank pain: Very possibly a urinary tract infection and/or kidney stone. You should get evaluated by your primary care doctor to determine what's going on & have the proper tests. If left untreated it could lead to sepsis (blood infection) and/or kidney failure.
