No: Although intercourse at time of ovulation is the best chance of getting pregnant, sometimes it does not occur, perhaps due to low sperm count, sticky cervical mucus or other causes.

No.: The time when you are ovulating is the best possible change to conceive, but keep in mind that everything has to be just right in order to actually get pregnant. A lot of times embryo will start, but implantation never occurs or implantation occurs but then spontaneously miscarry. The static is that if you have sex on the exact day that you ovulate there is a 1 in 4 chance of getting pregnant.
