Gradually is best: 15 months is a tough age to wean a baby, as they are extra clingy due to the normal separation anxiety at this age. I usually advise my patients to wait till 18 months if possible. If you must wean now, for a medical or other reason, then start by Remove just one feeding today and remove another every 2-3 days. The daytime ones are easiest at first as you can keep the toddler occupied.
