Just say it: Doctors and nurses have heard EVERYTHING. It's difficult to shock or surprise them with anything you might have. Just be honest and straight-forward about your symptoms. There is more trouble caused by people not speaking up, or "beating around the bush", and hinting about a problem, than just saying what the problem is straight out.
